Chronic Care Management: care that keeps going between visits. Here are three things to know. Chronic Care Management, or CCM, is a Medicare service for people with two or more serious chronic conditions expected to last at least a year, like arthritis and diabetes. CMS notes that it is typically provided outside of face-to-face visits. Whether it fits you is a decision you make with your provider. Under Medicare's rules, CCM includes a care plan with your health problems, goals and medicines, and a review of how you take those medicines. It adds support after an emergency room visit or a hospital stay, a consistent care team member who stays in touch, and ways to reach your team by phone and secure messaging. It starts with a visit with your provider. Before CCM begins, we explain the service, including any cost-sharing under your coverage, and ask for your consent. Only one practice can provide CCM for you in a month, so your care has one home base, and you can stop at any time. Read the full article at hopeintegrativemed.com, and ask us about enrolling.
