If you are new to Hope, or it has been more than a year since your last visit, a visit comes first. It can be a comprehensive visit, an annual wellness visit or an initial preventive physical exam.

Medicare covers Chronic Care Management for eligible patients, and many other plans may too. Cost-sharing applies, such as the Part B deductible and 20% coinsurance with Original Medicare, and we explain it before you start.
