What the guideline says

  • Chronic Care Management (CCM) is a Medicare service for people with two or more serious chronic conditions, like arthritis and diabetes, expected to last at least a year.
  • CMS notes that CCM services are typically provided outside of face-to-face visits, so care keeps moving between appointments.
  • Under Medicare's rules, CCM includes a comprehensive care plan, a medicine review, support when moving between care settings, a consistent care team member and phone and secure messaging access.
  • Depression and substance use disorders are among the conditions CMS lists that can count toward CCM.
  • Eligibility is a clinical decision your provider makes with you; having two diagnoses does not qualify you automatically.
  • CCM starts with a visit and your consent, only one practice can provide it in a calendar month, and you can stop at any time.
