Thyroid Disorder Management in the Charleston Area
Thyroid disorder management for adults in the Charleston area, in person at our West Ashley office and by telehealth.
Thyroid Conditions We Manage
- Hypothyroidism — Underactive thyroid; the most common thyroid condition, particularly in women. Managed with levothyroxine (Synthroid) titrated to TSH goal.
- Subclinical Hypothyroidism — Mildly elevated TSH with normal T4; evaluated individually for treatment based on symptoms, antibody status, and cardiovascular risk.
- Hashimoto's Thyroiditis — Autoimmune hypothyroidism; the most common cause of hypothyroidism in the US. Managed with thyroid replacement and monitoring.
- Established Hyperthyroidism Under Specialist Management — We coordinate with endocrinology for hyperthyroidism and provide primary care alongside specialty management.
Thyroid Management at Hope Integrative Medicine
Your initial evaluation includes TSH and free T4 labs (ordered if recent results are not available), a symptom review, and medication history. For patients already on levothyroxine who need a new prescriber, we can often continue your current dose and establish care at the first visit. For new diagnoses, we start treatment based on your lab values and symptom burden and monitor with follow-up labs at 6 to 8 weeks.
We prescribe levothyroxine (Synthroid, generic) and liothyronine (T3) for appropriate candidates. We coordinate with endocrinology when hyperthyroidism, thyroid nodules, or thyroid cancer management requires specialty involvement.
Thyroid Conditions and Mental Health
Hypothyroidism frequently presents with depression, fatigue, brain fog, and anxiety — symptoms that overlap significantly with primary psychiatric conditions. Many patients are treated for depression alone when an underlying thyroid disorder is the primary driver or a significant contributing factor. Hope Integrative Medicine's integrated model means thyroid function is evaluated as part of the full clinical picture, not in a separate silo from mental health care.
Prescriptions and Safety
Telehealth visits are evaluation and management visits. A prescription can be part of your treatment when your provider, using professional judgment and deciding together with you after weighing the risks and benefits, finds it medically necessary. Whether you are seen in person in West Ashley or by telehealth, prescriptions are sent electronically to your pharmacy.
Call 911 or go to the nearest emergency room for chest pain or pressure, trouble breathing, signs of stroke, a severe allergic reaction, or severe abdominal pain.

Frequently Asked Questions
Yes. TSH and thyroid labs can be ordered to a local laboratory and reviewed at a telehealth visit. Levothyroxine prescribing and dose titration, the core of hypothyroidism management, work well via telehealth. We offer in-person visits at our West Ashley office when preferred.
Yes, in most cases. If you have an established hypothyroidism diagnosis and current levothyroxine prescription, we can typically continue your medication and establish as your primary care provider at the first visit, subject to a clinical evaluation and recent or ordered labs.
Subclinical hypothyroidism (mildly elevated TSH with normal T4) is evaluated individually. Treatment decisions consider your TSH level, symptoms, thyroid antibody status, age, and cardiovascular risk. Some patients benefit from treatment; others are appropriately monitored without medication. We discuss the evidence and your individual situation at your visit.
Yes. Thyroid peroxidase antibody (TPO Ab) testing is appropriate when evaluating the cause of hypothyroidism, particularly in younger patients or those with fluctuating thyroid function. A Hashimoto's diagnosis doesn't change the core treatment — thyroid replacement — but informs monitoring and helps explain the clinical picture.
Possibly. Hypothyroidism can contribute to an incomplete response to antidepressants. If your thyroid function hasn't been checked recently, it should be. Hope Integrative Medicine's integrated model means we evaluate the physical and psychiatric picture together rather than in separate offices.
